Wellness Referral Center
The Healthy Here Wellness Referral Center (WRC) is operated by Adelante as part of our Contact Center operations. Presbyterian Healthcare Services Community Health, a founding partner and funder of the WRC, engages and trains participating clinics to make referrals using the system.
Medical providers recommend classes or programs to patients, for example healthy cooking classes for people with diabetes. Then the WRC contacts the patient to help them find and enroll in those classes. Once done, the WRC team follows up with the doctor. Together, we are improving the health of our community.
Wellness Referral Center programs and classes are available for free to participants, in part, through funding by Presbyterian Community Health and Presbyterian Healthcare Foundation. This work is also made possible thanks to funding from the Centers for Disease Control & Prevention.

The Wellness Referral Center registers participants in local activities to support a healthy lifestyle. For example: physical activities, nutritious cooking classes, food access, and chronic disease self-management classes.
Referring Medical Providers
Qualified clinics and healthcare providers refer participants to the WRC and our resources. Participation information is sent back to the referring provider so activities can be tracked. Click here to learn about the activities the program offers and to download referral forms.
Rewards for Improving Health
People who attend classes to improve their health can also earn rewards. Quarterly drawings are held and people win prizes such as activity trackers from Presbyterian’s Center for Community Health or recycled laptops from Adelante’s DiverseIT program.
